Please don't send Social Security numbers, bank or card numbers, or medical records through our website chat or email. If we need sensitive information to help with an application, we'll use an appropriate secure process.

Our website and services are not directed to children, and we do not knowingly collect personal information from children under 13.

We may also share information when you ask us to or give us permission. The text-messaging protections in sections 9 and 10 apply to all sharing.

As required by Medicare rules, sales and enrollment calls may be recorded and kept on file; we'll tell you at the start of a call if it's being recorded.

Our website may use cookies and similar technology to operate the site, power our chat feature, remember preferences and understand how the site is used. This may include your IP address, browser and device type, referring website, pages visited, approximate location based on IP address, and the date and time of your visit.

Our site links to insurance companies, government resources such as Medicare.gov and SHINE, our Facebook page and community partners. We don't control those sites, and information you give them is covered by their own privacy policies. A link isn't an endorsement of everything that organization offers.

9. Text message consent

When you give us your mobile number and opt in — through our website chat or by written or verbal permission — you agree to receive text messages from Curbello Health Consultant. Messages may include answers to your questions, appointment scheduling and reminders, follow-ups, and information about Medicare and insurance coverage options.

Message frequency varies. Message and data rates may apply. Reply STOP to opt out or HELP for help. Consent to receive text messages is not a condition of any purchase.

10. Mobile information

No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text messaging originator opt-in data and consent are excluded from all information sharing described in this policy and will not be shared with or sold to any third party.

Your number may be used by the service providers that deliver our messages, such as our texting platform and wireless carriers, only to operate our messaging program. Carriers are not liable for delayed or undelivered messages.

11. Opting out

You can opt out at any time by replying STOP to any message. You'll receive one message confirming you've been unsubscribed and no further texts after that. To rejoin, text START or opt in again. 12. Independent insurance agency

Curbelo Healthcare LLC, doing business as Curbello Health Consultant, is an independent insurance agency based in Palm Beach County, Florida. Insurance products are offered by Joshua Geribon, a licensed insurance agent in the State of Florida, License No. G114712. We are licensed in Florida only.

Products, insurance companies, eligibility and services vary by location and individual circumstances. Insurance-company availability varies by product, location, eligibility and appointment status.

Compensation: there is no cost for a consultation with us. When you enroll in a plan or buy a policy through us, we may be paid a commission by the insurance company. You don't pay us a separate fee, and premiums are set by the insurance company.

13. Medicare disclosure

We do not offer every plan available in your area. Currently we represent 4 organizations which offer 8 products in your area. Please contact Medicare.gov, 1-800-MEDICARE, or your local State Health Insurance Program (SHIP) to get information on all of your options. Plan availability varies by ZIP code.

Curbello Health Consultant is not connected with or endorsed by the U.S. government, the federal Medicare program, Medicaid, the Centers for Medicare & Medicaid Services or the U.S. Department of Veterans Affairs. In Florida, free Medicare counseling is available through SHINE.

Premiums, benefits, provider networks, drug lists, pharmacies and cost-sharing vary by plan and service area. Enrollment depends on eligibility, enrollment periods and plan contract renewal.

14. Educational information

Information on this website is general and educational. It is not legal, tax, investment, financial or medical advice, and product descriptions are summaries — not complete statements of coverage. The policy, Evidence of Coverage and official insurance-company documents control the actual benefits, limitations and exclusions.

Insurance and annuity products are subject to eligibility, suitability, underwriting where applicable and insurance-company approval. Annuities are long-term products that may have surrender charges and are not FDIC insured. Guarantees are backed by the financial strength and claims-paying ability of the issuing insurance company. 15. Community events

Our community events are open to everyone. Attending never requires you to share contact information, discuss insurance or enroll in a plan. Educational Medicare sessions do not include sales or enrollment.
